Unity Care Companions Program Available to All Adults Needing Assistance at Home

Unity Care Companions has been serving clients in Green Bay communities since August of 2021. The program is designed to provide individualized services that promote the independence of any adult wherever they call home.

Care Companions offers a menu of nonmedical services including assistance with personal cares, light housekeeping, companionship, meal preparation, medication reminders, cognitive support, home safety assessments with recommendations, and transportation.

Unlike home health services, a physician’s order is not required and there are no health requirements for eligibility. You do not need to be enrolled in any other Unity program to have assistance from Care Companions. 

Anyone who needs assistance at home can be referred for a complementary assessment.

A referral to Care Companions may be appropriate for those who face challenges with:

Medication Management – taking medications as scheduled/prescribed

Personal Care & Hygiene – bathing, grooming, a decrease in personal hygiene, or choosing weather appropriate clothing

Housekeeping –keeping up with routine housekeeping tasks, changing the linens, doing the laundry/dishes, keeping organized, etc.

Nutrition  – shopping for and preparing well-balanced meals

Behavior/Mental Health – feelings of loneliness, isolation, boredom, anxiety, depression, or confusion/memory

Mobility – walking, getting up from a seated position, or falls

Medical Condition – a new or existing condition or progressing illness that limits your independence
